I agree to the Championship RULES and CONDITIONS and acknowledge personal receipt of a printed copy thereof from the SPMA website.  I agree that there are certain risks inherent in the game of golf and accept personal and sole liability for all such risks.  I understand that all eligible players desiring to play in the Championship must submit an entry form, or online entry, that is received by the SPG on or prior to the entry deadline, and that all entries are subject to acceptance, as disclosed in the SPMA Rules and Conditions.

Enter minimum of 5 digits/letters of your Handicap/Index number identifying your handicap/Index and we, and we only, will verify such handicap/Index.

< all specific stipulated invitation criteria for the SPSA are disclosed

and listed on the Eligibility webpage of this SPSA website. 

If none of such criteria apply in your case, you are not invited.  If you enter the SPSA claiming to be invited when, in fact, you are not, any payment of entry fees via PayPal will be treated

as an interest free loan to the SPSA until October 31 and the amount returned to you at that time will be net of the $20 withdrawal fee applicable to entering the SPSA online.

Notice - if you are unable or unwilling to provide all info required

in this online entry form, then you must complete a hardcopy SPC entry form and mail such entry with a $225 check to the SPMA so as to arrive on or prior to the entry deadline - no exceptions.

If you enter the SPC online before the entry deadline but after all allocated exempt spots in the field have filled, then you will be placed on a waiting list pending withdrawals of other entrants who entered before you.
